Support Team

Team-based care for complex mental health needs, crisis prevention, and community support.

What is CST?

Community Support Team (CST) is an intensive, team-based mental health service designed for individuals with complex needs who require ongoing, comprehensive care. Delivered under the direction of a full-time Qualified Mental Health Professional (QMHP), CST is available 24/7, 365 days a year. It is intended to reduce behavioral health crises, prevent unnecessary hospitalizations, and help individuals achieve recovery and resilience.

How does CST work?

CST uses a multidisciplinary team approach to provide a full range of mental health support. This includes direct treatment, restorative interventions, case management, and crisis intervention. While team members may deliver specific services, they work together as a single, coordinated unit. The Team Lead provides direct clinical interventions, ensuring care is both consistent and responsive to each person’s needs.

Why is CST important?

For individuals at higher risk of crisis or hospitalization, CST offers a lifeline of consistent and coordinated care. By addressing mental health challenges proactively and intensively, CST helps individuals stay connected to their communities, avoid institutional settings, and improve their overall quality of life. The team’s around-the-clock availability ensures that help is always within reach—whenever it’s needed.

What are the goals of CST?
  • Reduce behavioral health crises and hospitalizations
  • Support harm reduction strategies
  • Increase daily functioning and independence
  • Connect individuals to community resources
  • Foster resilience and recovery through ongoing, coordinated care
